The introduction of the dental operating microscope in 1981 was a turning point in the history of dentistry, triggering a rapid transition from the conventional world of macrodentistry to the precise, detailed world of microdentistry. This paradigm shift is evident in almost all dental specialties, but particularly in endodontics, periodontics, restorative dentistry, and dental technology. This article traces the development and acceptance of the operating microscope from the introduction of primitive magnifying tools and the early beginnings of microsurgery to the arrival of the modern sophisticated instrument. For the dental office of today, the dental operating microscope is a state-of-the-art diagnostic and therapeutic tool, with significant clinical applications in both surgical and nonsurgical dental specialties. Microscope-assisted precision dentistry, with its increased clinical success rates, has set a new standard in patient care.
